Haiti: Public Health and Structural Change Auteur(s) : Ivers, Louise Éditeur(s) : Center for the Humanities and the Public Sphere, University of Florida Center for the Humanities and the Public Sphere, University of Florida ( Gainesville, FL ) Résumé : (Biographical) Dr. Louise Ivers is Chief of Mission for Partners In Health (PIH) in Haiti, an international non-profit organization that provides direct health care and social services to poor communities around the world, supported by research and advocacy. She is an Assistant Professor of Medicine at Harvard Medical School and an Associate Physician in the Division of Global Health Equity at Brigham and Women's Hospital (BWH). She completed her residency in Internal Medicine at Massachusetts General Hospital (MGH) and a fellowship in Infectious Diseases at the combined MGH/BWH program. Dr. Ivers also received a diploma in Tropical Medicine and Hygiene from the London School of Hygiene & Tropical Medicine and a Master of Public Health from the Harvard School of Public Health. Dr. Ivers implements health programs, and is interested in improving the delivery of healthcare in resource poor settings, the provision of care to the rural and urban poor, as well as patient-oriented investigation that offers solutions to barriers to healthcare. She balances her time between management of PIH Haiti, direct clinical service, and operational research. Dr. Ivers has contributed to published articles on HIV/AIDS, food insecurity, and the prevention of mother-to-child transmission of HIV. Current projects include investigating the impact of targeted food assistance to people with HIV infection in rural Haiti, the effectiveness of ARV therapies on viral suppression in community-based programs, and humanitarian assistance in response to the January 2010 earthquake. Dr. Ivers has served as a Technical Advisor to the WHO and also mentors Haitian and American physicians. Bryant Foundation, where books, music and art indigenous to the regions were gathered. The are organized by geographical location. Two men, Berkeley F. Jones (wearing a white hat) and a travel companion, look at houses in this Rock Sound neighborhood on Eleuthera, Bahamas. Éditeur(s) : HAL CCSD Elsevier Résumé : International audience Chlordecone is a persistent organochlorine pesticide that was used in the French West Indies until the early 1990s for banana weevil borer control. Human exposure to this chemical in this area still occurs nowadays due to consumption of contaminated food. Although adverse effects on neurodevelopment, including tremors and memory deficits, have been documented in experimental studies conducted with rodents exposed during the gestational and neonatal periods, no study has been conducted yet to determine if chlordecone alters child development. This study examines the relation of gestational and postnatal exposure to chlordecone to infant development at 18 months of age in a birth-cohort of Guadeloupean children. In a prospective longitudinal study conducted in Guadeloupe (Timoun mother-child cohort study), exposure to chlordecone was measured at birth from an umbilical cord blood sample (n=141) and from a breast milk sample collected at 3 months postpartum (n=75). Toddlers were assessed using an adapted version of the Ages and Stages Questionnaire. Higher chlordecone concentrations in cord blood were associated with poorer fine motor scores. When analyses were conducted separately for boys and girls, this effect was only observed among boys. These results suggest that prenatal exposure to chlordecone is associated with specific impairments in fine motor function in boys, and add to the growing evidence that exposure to organochlorine pesticides early in life impairs child development. Artist Everald Brown was a regular art exhibitor, often accompanied by his son Clinton, a fellow artist. The walls of his studio, pictured, are cover with paintings and photographs. One black and white photograph features musicians and another is of the Madonna and Child. This study sought to examine the effects of solvent exposure during the prenatal and postnatal periods on wheezing, eczema and food allergies in early childhood. METHODS: This study, based on the PELAGIE cohort, included 1505 mother-child pairs with measurements of prenatal and postnatal solvent exposures and data on wheezing, eczema or food allergies. The maternal occupation reported at inclusion, in early pregnancy allowed us to define prenatal occupational solvent exposure (by three specific job-exposure matrices). Data on prenatal and postnatal domestic solvent exposure, that is exposure to products that contain solvents, were obtained from self-administered questionnaires, once at inclusion and again when the child was 2 years old. Outcome data was collected at the 2-year follow-up. Associations between exposures and outcomes were estimated by logistic regression models, after adjustment for potential confounders. RESULTS: No association was observed between prenatal exposure to solvents and the outcomes studied. Postnatal exposure was associated with an increased risk of wheezing (OR=1.80 (95% CI 1.25 to 2.59)) which persisted after adjustment for prenatal exposure. No significant association was observed with eczema or food allergies. CONCLUSIONS: Postnatal exposure to solvent-containing products in the home may increase the risk of wheezing in early childhood. Follow-up studies are needed to determine if the health effects observed at age 2 persist at later ages.
