OBJECTIVE: We examined if the CNS Penetration-Effectiveness (CPE) score of antiretroviral drugs was associated with survival after a diagnosis of HIV-related encephalopathy, progressive multifocal leukoencephalopathy (PML), cerebral toxoplasmosis, or cryptococcal meningitis. METHODS: Using data from the FHDH-ANRS CO4, we compared the survival of 9,932 HIV-infected patients diagnosed with a first neurologic AIDS-defining event in the pre-combination antiretroviral therapy (cART) (1992-1995), early cART (1996-1998), or late cART (1999-2004) periods. Follow-up was subdivided (CPE < 1.5 and CPE ≥ 1.5), and relative rates (RR) of death were estimated using multivariable Poisson regression models. RESULTS: In the pre-cART and early cART periods, regimens with CPE ≥ 1.5 were associated with lower mortality after HIV-related encephalopathy (RR 0.64; 95% confidence interval [CI] 0.47-0.86 and RR 0.45; 95% CI 0.35-0.58) and after PML (RR 0.79; 95% CI 0.55-1.12 and RR 0.45; 95% CI 0.31-0.65), compared to regimens with CPE < 1.5, while in the late cART period there was no association between the CPE score and the mortality. A higher CPE score was also associated with a lower mortality in all periods after cerebral toxoplasmosis (RR 0.68, 95% CI 0.56-0.84) or cryptococcal meningitis (RR 0.50, 95% CI 0.34-0.74). Whatever the neurologic event, these associations were not maintained after adjustment on updated plasma HIV-RNA (missing, <500, ≥500 copies/mL) with RR ranging from 0.82 (95% CI 0.36-1.91) to 1.02 (0.69-1.52). CONCLUSION: At the beginning of the cART era, the CPE score was of importance for survival after severe neurologic event, while in the late cART period, the additional effect of CPE score vanished with more powerful antiretroviral regimens associated with plasma viral load control.

Résumé : The New Caledonia blue shrimp Litopenaeus stylirostris is commercially produced under semi-intensive rearing conditions. The size of the farming earthen ponds (5–10 ha) and the subsequent production constraints make it difficult to use them as experimental units for random experiments. Moreover, since every pond has its own characteristics, ponds' effects cannot be ruled out, thus making it hard to define true replicates. In order to design future experiments under conditions resembling those used in production, we evaluated the possible use of floating cages as experimental units with the aim of assessing treatment effects with a reasonable statistical power. To this end, two sets of floating cages were placed respectively in two different ponds in a commercial farm. In both cases the zootechnical conditions in the cages were similar in post-larvae origin, management, and diet and feeding regime. The aim of the study was to evaluate on a technical basis the possibility of rearing shrimps in floating cages set up in earthen ponds and to assess this method from a statistical standpoint. Shrimps reared in and outside the cages showed comparable growth and survival rates. The variability in the zootechnical parameters between cages, expressed as the estimated coefficient of variation (CV) for survival rate, total final biomass (g), final body weight (g), daily increment in body weight (g day− 1) and FCR were 11.0, 13.7, 4.4, 5.4, and 7.0%, respectively. On the basis of these figures, we calculated for a given statistical power (80%) the number of replicates (cages) that would be required to reveal significant differences between two treatments, at a 5% level of significance. We found that for expected differences of 20% from the control mean, 3 and 6 floating cages per treatment would be reasonable to determine statistical differences for growth parameters and survival rate, respectively. Moreover, we showed a significant pond effect in regard to survival and growth between the two sets of cages. These results illustrated the within-farm variability among the ponds, and confirmed that the specific characteristics of each pond from the same farm make it difficult to use the ponds themselves as experimental units. The study demonstrates that rearing in floating cages is an economical, powerful and sensitive experimental tool for shrimp culture studies specifically carried out under conditions close to semi-intensive production.

Résumé : Bonamiosis is a parasitic disease (causative agent: Bonamia ostreae) affecting the European flat oyster Ostrea edulis, responsible for a drastic decline in the aquaculture production of this oyster species. Therefore a selective breeding program for resistance to bonamiosis has been undertaken since 1985 bu Ifremer, leading to the production of several selected oyster families. In the present study, a 6-month cohabitation challenge experiment was performed in order to transmit the disease from wild oysters (injected with the parasite) to two tested families of oysters originating from the selective breeding program. Mortalities were checked daily, and ventricular heart smears were performed on dying or moribund oysters to detect the level of infection to B. ostreae. The first infections occurred after 4 months of challenge in the tested oysters (Family 1 and Family 2). The cumulative mortalities after 5 monts were 58% for the wil oysters, 9% for Family 1 (20-month old at the beginning of the experiment) and 20% for Family 2 (8-month, old). The parasite could be detected in 66.8% of the dying wild oysters, 67.5% of the dying oysters of Family 1, 89% of the dying oysters of Family 2 and only 11% of the surviving oysters of Family 2. The mortality was significantly higher in Family 2 thant in Family 1 (x2= 20.87, p<0.001, d.f.) as well as the level of infection by the parasite found in heart smear (x2=24.34, p<0.001, 4 d.f.). This result demonstrates that prespawning oysters as yong as 1 year-old can become infected with the parasite and die from bonamiosis. This result is inconsistent with the commonly accepted critical age of 2 years-old for the disease development. The most probable cause of the dscrepancy in the development of bonamiosis between the 2 tested families is a difference in genetic background.

Résumé : We investigated performance trade-offs among seedlings of nine tropical tree species during a five-year field experiment. Seedlings were grown in eight microhabitat types composed of paired gap and shaded understory sites in each of four soil types. We defined performance trade-offs relevant to coexistence as significant pairwise rank reversals for species performance between contrasting situations, of which we characterize three types: microhabitat, fitness component, and ontogenetic. Only 2 of 36 species pairs exhibited microhabitat trade-offs or reversed rankings for survival or relative growth rate (RGR) among microhabitats, and only one species pair reversed performance ranks among soil types. We found stronger evidence for rank reversals between fitness components (survival and RGR), particularly in gap vs. understory environments, suggesting a general trade-off between shade tolerance (survival in shade) and gap establishment (RGR in gaps). Third, the most frequent rank reversals between species pairs occurred between early and later ontogenetic stages, especially between fitness components in contrasting microhabitats. Overall, 15 of 36 pairs of potentially competing species exhibited some type of seedling performance trade-off, two species pairs never outperformed one another, and for 19 species pairs one species was a consistent better performer. We suggest that ontogenetic trade-offs, in concert with microhabitat and fitness component trade-offs, may contribute to species coexistence of long-lived organisms such as tropical trees.

Purpose: Active surveillance is a treatment option for favorable risk prostate cancer. However, data are missing on populations of African descent. We evaluated the safety and benefit of active surveillance in an African Caribbean cohort with favorable risk prostate cancer. Materials and Methods: Between 2005 and 2016, a single center, prospective cohort study was performed in Guadeloupe, French West Indies, including patients on active surveillance who had low risk prostate cancer (prostate specific antigen 10 ng/ml or less and Gleason score 6 or less) or favorable intermediate risk prostate cancer (prostate specific antigen 10 to 20 ng/ml, Gleason score 3 + 4 or less and life expectancy less than 10 years). Treatment was recommended in case of grade progression, increased tumor volume, prostate cancer doubling time less than 36 months or patient wish. Overall survival, disease specific survival and duration of active surveillance were calculated with the KaplanMeier method. Multivariate analysis was performed using the Cox proportional hazards model to identify predictors of active surveillance termination. Results: A total of 234 patients with a median age of 64 years were enrolled in study. Median followup was 4 years (IQR 2.3-5.5). Overall survival at 30 months, 5 years and 10 years was 99.5%, 98.5% and 90.7%, respectively. Disease specific survival at 30 months, and 5 and 10 years was 100%. At 30 months, 5 years and 10 years 72.7%, 52.6% and 40.4% of patients, respectively, remained untreated and on active surveillance. Age (HR 0.96 per additional year, 95% CI 0.93-0.99) and prostate specific antigen density (HR 1.52 per additional 0.1 ng/ml, 95% CI 1.20-1.89) were found to be independent predictors of active surveillance termination. Conclusions: Active surveillance is safe and beneficial for highly selected African Caribbean patients. It seems to be feasible for patients at low risk and intermediate favorable risk. Prostate specific antigen density could help better select these patients.

Description : In the Global South, improvement of agricultural outputs is eagerly awaited. While by 2050, its population will double areas devoted to agriculture will decrease exacerbating undernutrition of the poor. Unfortunately, the demand for fresh locally-produced meat products is not satisfied yet in the tropics. So, efficiency in animal productions is essential to allow coverage of protein nutritional needs of people, both in quality and quantity. In the Caribbean territories mixed farming systems are the most common farming systems (about 80%) and can constitute a solution to reach food sovereignty in such limited and isolated spaces. Lessons to be learned from these systems involve improving animal performance while respecting the natural balance with environment and maintaining the multi-functionality of plants and animals. Firstly, the natural (or selected) comfort zone of animal and plants must fit with the farm conditions, insuring thereby animal survival and welfare. Enhancement of adaptation to stresses (biotic, abiotic and socio- economic) in species or animal genotypes is a key element implying their equilibrium with the farm environment. This approach underpins for the farmer an integrated management of animal health, nutrition, genetics, reproduction, in close relationship with other compartments of the farming system. Secondly, animal adaptation may be completed by resilience ability within systems. Animals have to produce although facing stresses. Finally, the animal must be efficient that is to say must reconcile physiological functions of production, reproduction with adaptation functions. This optimization leads to decreased inputs and to overall efficiency of mixed farming systems at the end. The aim of the breeder is to choose the animal producing the best balance between output-reproduction-adaptation, at the individual or the flock scale combining genetic and physiological diversity. The underlying idea is to give to humans and animals their right place in the food chain taking into account the farmers? skills and wills. This idea is included in the agroecological approach and may give guidelines for food sovereignty worldwide.
