Aims: The aims of this study were to compare short-and long-term outcomes for clinical T2N0 oesophageal cancer with analysis of (i) primary surgery (S) versus neoadjuvant therapy plus surgery (NS), (ii) squamous cell carcinoma and adenocarcinoma subsets; and (iii) neoadjuvant chemoradiotherapy versus neoadjuvant chemotherapy.Methods: Data were collected from 30 European centres from 2000 to 2010. Among 2944 included patients, 355 patients (12.1%) had cT2N0 disease; 285 (S) and 70 (NS), were compared in terms of short-and long-term outcomes. Propensity score matching analyses were used to compensate for differences in baseline characteristics.Results: No significant differences between the groups were shown in terms of in hospital morbidity and mortality. Nodal disease was observed in 50% of S-group at the time of surgery, with 20% pN2/N3. Utilisation of neoadjuvant therapy was associated with significant tumour downstaging as reflected by increases in pT0, pN0 and pTNM stage 0 disease, this effect was further enhanced with neoadjuvant chemoradiotherapy. After adjustment on propensity score and confounding factors, for all patients and subset analysis of squamous cell and adenocarcinoma, neoadjuvant therapy had no significant effect upon survival or recurrence (overall, loco-regional, distant or mixed) compared to surgery alone. There were no significant differences between neoadjuvant chemotherapy and chemoradiotherapy in short-or long-term outcomes.Conclusion: The results of this study suggest that a surgery alone treatment approach should be recommended as the primary treatment approach for cT2N0 oesophageal cancer despite 50% of patients having nodal disease at the time of surgery.

Résumé : Sites of monomethylmercury (MMHg) production in Amazonian regions have been identified in hydraulic reservoirs, lake sediments and wetlands, but tailings ponds have not yet received sufficient attention for this purpose. This work evidenced high MMHg production within the water column and the interstitial water of two tailings ponds of French Guiana Au mines located; (i) in a small scale exploitation (Combat) where Hg was used for Au amalgamation, and (ii) in an industrial on-going Au mine (Yaoni) processing without Hg. The (MMHg)(D) maximum (2.5 ng L-1) occurred in the oxic water column above the sediment-water interface (SWI) of the most recent tailings pond (Combat), where the substrate was fresh, the redox transition was sharp and the pool of total Hg was large. In the Yaoni pond, the (MMHg)(D) maximum concentration (1.4 ng L-1) was located at the SWI where suboxic conditions prevailed. Using the (MMHg)(D) concentration as a proxy for Hg methylation rates, the present results show that Hg methylation may occur in various redox conditions in tailings ponds, and are favored in areas where the organic matter regeneration is more active. A 3-month long laboratory experiment was performed in oxic and anoxic boxes filled with high turbidity waters from the Combat Au mine to simulate tailings ponds. Slaked lime was added in an experimental set (2 mg L-1)and appeared to be very efficient for the reduction of suspended particulate matter (SPM) to environmentally acceptable concentrations. However, at the end of the experiment, large (MMHg)(D) concentrations were monitored under treated anoxic conditions with the (MMHg)(D) maximum located at the SWI above the Fe-reducing zones. No (MMHg)(D) was detected in oxic experiments. The use of slaked lime for SPM decantation appears to be an efficient and non-onerous process for Au miners to avoid Hg methylation in tailings ponds when it is combined with rapid drainage of the mine waters. A subsequent human intervention is however necessary for the recovery of soil structure through the cover of dried ponds with organic rich materials and reforestation to avoid the stagnation of rain waters and the occurrence of anoxia. Résumé : This report synthesises the knowledge on the biogeochemical cycle, bioconcentration processes and ecotoxicity of lead in the marine environment. Man induced changes on the lead cycle, especially along the French coasts are reviewed. Concentration levels, mechanisms and fluxes between geochemical reservoirs are assessed. Emphasis is given on sublethal effects and toxicity mechanisms. Quality standards used in European Community countries are listed. Conclusions and recommendations for environmental management and research are given.
Le présent document constitue une synthèse des connaissances sur le cycle biogéochimique, la bioconcentration et l'écotoxicité du plomb en milieu marin. Il fait état des perturbations du cycle du plomb par l'action de l'homme, en particulier dans le milieu littoral français. Les niveaux de concentration et les quantités échangées entre les réservoirs géochimiques sont évalués. L'accent est mis sur les effets sublétaux et les mécanismes de toxicité vis-à-vis du biotope marin. Les réglementations relatives en milieu marin actuellement en vigueur dans les pays de la Communauté européenne sont répertoriées. Avec les conclusions quelques recommandations en matière de gestion et de recherche sont faites.

Résumé : Le présent rapport constitue une première présentation des résultats de l'Etude pilote des apports en contaminants par la Seine. Il est consacré aux métaux et à l'arsenic et traite de leurs niveaux de concentration, de leurs variations saisonnières et des flux à l'estuaire. Il comporte des fiches-annexes qui contiennent les méthodes de prélèvement, de filtration et de dosage ainsi que des résumés des résultats. A partir des conclusions, des recommandations sont faites quant aux méthodes à privilégier pour tirer les meilleurs enseignements du suivi des contaminants métalliques et de l'arsenic à la limite amont de l'estuaire de la Seine. Les données contenues dans ce rapport ont été produites par la Cellule de lutte contre la pollution (Service de navigation de la Seine), le laboratoire Chimie des contaminants et modélisation (IFREMER) et le Laboratoire municipal et régional de Rouen.

Résumé : The distribution and speciation of mercury (Hg) in air, rain, and surface waters from the artificial tropical lake of Petit-Saut in French Guiana were investigated during the 2003/04 period. In the air, total gaseous mercury (TGM) at the dam station averaged 12 +/- 2 pmol m(-3) of which > 98% was gaseous elemental mercury (GEM). GEM distribution depicted a day-night cycling with high concentrations (up to 15 pmol m(-3)) at dawn and low concentrations (down to 5 pmol m(-3)) at nightfall. Reactive gaseous mercury (RGM) represented < 1% of the GEM with a mean concentration of 4 3 fmol m(-3). Diel RGM variations were negatively related to GEM. In the rain, the sum of all Hg species in the unfiltered (HgTUNF) averaged 16 +/- 112 pmol L-1. Temporal distribution of HgTUNF exhibited a pattern of high concentrations during the late dry seasons (up to 57.5 pmol L-1) and low concentrations (down to 2.7 pmol L-1) in the course of the wet seasons. Unfiltered reactive (HgRUNF), dissolved gaseous (DGM) and monomethyl (MMHgUNF.) Hg constituted 20, 5 and 5% of HgTUNF, respectively. All measured Hg species were positively related and displayed negative relationships with the pH of the rain. In the reservoir surface waters, dissolved total mercury (HgTD) averaged 3.4 +/- 1.2 pmol L-1 of which 10% consisted of DGM. DGM showed a trend of high concentrations during the dry seasons (480 +/- 270 finol L-1 and lower (230 +/- 130 finol L-1) in the course of the wet seasons. Diel variations included diurnal photo-induced DGM production (of about 60 firiol L-1 h(-1)) coupled to minute to hour oxidation/ reduction cycles (of > 100 firiol L-1 amplitude). Finally, calculated atmospheric Hg inputs to the Petit-Saut reservoir represented 14 mot yr(-1) whereas DGM evasion reached 23 mol yr(-1). Apportionment among forms of Hg deposition indicated that up to 75% of the total Hg invasive flux follows the rainfall pathway. (C) 2007 Elsevier B.V. All rights reserved.

A sediment core encompassing 3500 years of continuous sedimentation has been collected from a coastal lagoon located on the southwestern French Mediterranean coast. Lead concentrations and stable isotopes show that the sediments have recorded the three major periods of Pb pollution: the Etruscan-Greek-Roman period (650 BC to AD 50), the medieval period (AD 650 to AD 1450), and the modern period (from around AD 1850 to the present). These periods were separated by low pollution periods during the Dark Ages (between AD 50 and 650) and during the 16th century. From the end of the 19th century to the 1960s, Pb pollution increased exponentially. Coal combustion was the major source of Pb in the lagoon in the second half of the 20th century. Both the decrease in coal consumption and the ban on leaded gasoline resulted in a decrease in Pb pollution by a factor of 1.5 between 1973 and 1995. From 1991, sewage treatment plants and incinerators could be the major source of Pb. The average baseline Hg concentration from 1525 BC to AD 900 was 0.017 ± 0.003 μg g(-1) (n = 54). The Hg concentrations profile shows three major peaks: in AD 1150, AD 1660, and AD 1969, with the concentrations being respectively 8, 5, and 34 times higher than the baseline levels. The medieval peak (AD 1150) is attributed the medical use of Hg in the town of Montpellier and/or the burning of soil and vegetation. Noticeable Hg pollution was also detected during the 17th century in relation to gold and silver amalgamation in Europe. From the end of the 19th century, Hg concentrations increased exponentially until 1969. This modern pollution is attributed to the burning of coal.

Résumé : Dissolved and particulate Hg fluxes in the Lot-Garonne-Gironde fluvial-estuarine system were obtained from observation of daily discharge and suspended particulate matter (SPM) concentrations. In addition to the measurements of the total dissolved (< 0.45 gm) and particulate Hg (> 0.45 gm), called HgTD and HgTp respectively, the dissolved inorganic Hg species (HgRD) were determined monthly. Geochemical background values for HgTp in sediments and SPM were similar to crustal values and to typical concentrations in SPM of non-contaminated river systems, respectively. The Riou Mort watershed already known as the origin of important historical polymetallic (e.g., Cd, Zn) pollution was identified as an important Hg point source. In the downstream Lot River, Hg concentrations were clearly higher than those in other moderately contaminated systems. The mean relative contribution of HgRD to HgTD in the Lot River and in the Garonne River was close to 25% and 50%, respectively, and showed no correlation with water discharge or SPM concentration. Depending on the origin and nature of SPM, HgTp concentrations were correlated or not with particulate organic C (POC). Maximum HgTp concentrations were measured in samples containing low POC concentrations and were attributed to sediment resuspension. In contrast, high POC concentrations (6-17%) during algal blooms were associated with low/moderate HgTp concentrations (< 0.5 mg kg(-1)) at different sites, suggesting that Hg concentrations in fluvial phytoplankton may be limited by bioavailability of dissolved Hg and/or physiologically controlled Hg accumulation. Mercury was mostly (up to 98%) transported in the particulate phase with estimated annual Hg fluxes at the outlet of the Lot River system ranging from 35 to 530 kg a(-1) for the past decade. The minimum anthropogenic component (58-84% of total Hg fluxes) could not be explained by present Riou Mort point source contributions, suggesting important Hg release from contaminated sediment as a major source and from downstream point sources (e.g., coal-fired power plants and/or metal processing industries). HgTp concentrations and fluxes were strongly related to hydrologic variations and were clearly increased by riverbed dredging during lock construction. Therefore, the estimated Hg stocks in the Lot River sediment (5-13 tons) represent an important potential Hg source for the downstream fluvial-estuarine system.

Résumé : Consumption of marine organisms represents one of the main exposure sources of contaminants for human populations. To obtain a global view of the contamination in commercial fish in the NW Mediterranean Sea, we analysed four types of priority contaminants (PCBs, PBDEs, Hg and Cs-137) in the European hake, Merluccius merluccius, from the Gulf of Lions in relation with organism's trophic level (delta N-15). All contaminants presented a significant increase in concentration in hake muscle with trophic level. However, obvious differences between contaminants were evidenced. Biomagnification factors (BMF and FWMF) along the hake food web were higher for Hg and CB-153 than for BDE-47 and Cs-137, and increase in contaminant concentration with trophic level occurred at different rates depending on contaminants. Such differences of biomagnification patterns can be related to physico-chemical properties of the different contaminants. (C) 2012 Elsevier Ltd. All rights reserved.

Résumé : The accumulation of nine heavy metals in fine-grained sediments from the mangrove fringed coast of French Guiana is evaluated. The dynamic features of the South American tropical coastline, from the Amazon to the Orinoco Rivers, result in mangrove sediments being alternately submitted to phases of erosion and net sedimentation a few tens of years long. This process influences the distribution of the heavy metals associated with these frequently re-mobilized deposits. Sedimentary cores and mangrove plant samples were collected, at different seasons, in various swamps characterized by different properties (content of sedimentary organic matter, distance from sea water and fresh water). The ranges of measured concentrations expressed in mu mol g(-1) were the following: Cu (0.06 to 0.61), Co (0.12 to 0.68), Pb (0.08 to 0.18), Ni (0.32 to 0.76), Cr (0.61 to 1.40), Zn (1.25 to 5.94), Mn (4.36 to 45.4) and Fe (441 to 1128). No differences were found between sediments from mangroves developing upstream and downstream of urban areas, i.e. Cayenne and Kourou. This suggests that the content of mangrove sediments in heavy metals along the coastline of French Guiana is essentially the result of the continuous alternation of accumulation and transport phases occurring upstream after departing from the Amazon watershed. The sources of this heavy metals content are thus difficult to identify. However it is well known that the alluvium produced by the natural erosion of the Amazonian soils is naturally enriched in mercury. Also, the run-off from gold mining activities is known to contribute to mercury pollution. Ranges in total Hg were between 0.15 and 2.57 nmol g(-1), with mean values close to 0.41 nmol g(-1), and were clearly correlated with total organic carbon except for some outstanding high values, which may be a result of rapid geochemical changes. Heavy metal concentrations showed variations with depth. The redox conditions and the decay processes affecting the organic matter control the cycling of iron and manganese, which in turn control the concentrations and associations of heavy metals. These preliminary results suggest that the variations in heavy metal content with depth or between mangrove areas result largely from diagenetic processes rather than changes in metal input resulting from local human activities. Résumé : We present results obtained from a pristine site of French Guiana (South America), at the Pic Matecho, a region far from gold mining activity. This area contains a small permanent lake, a rare situation in French Guiana. Tree canopy, soils and Lake Matecho sediments were analysed in total Hg and Pb-210. The annual deposit of litterfall is estimated at 40 mug (Hg)/m(2)/y. The top sediment content of 0.2 mug (Hg)/g indicates a large contribution of the atmospheric deposit. In sediment, the decrease of total Hg with depth is neither due to local human influence, nor to increase of atmospheric background during this century. It is attributed to remineralization of organic matter in the upper part of the sediment : the majority of the total Hg deposited at the water/sediment interface is not permanently trapped in the sediment but remobilized in water. Interpretation of the historical sediment profile suggests a constant deposit rate over the last millennium.

Résumé : The distribution, partition and speciation of mercury (Hg) were studied along the redox gradient of an anthropogenically perturbed tropical estuary, the Sinnamary Estuary in French Guiana. This system is a partially mixed estuary characterized by an anoxic freshwater end-member, while the marine end member consists of the Amazon Plume. The set up of an artificial oxygenation system in the anoxic freshwater end-member generates sharp gradients of major chemical species (iron, sulfides, etc.) coupled with intense organic matter (OM) turnover. The coexistence of oxygenated waters and dissolved sulfides in an organic rich environment depicts the Upper Sinnamary Estuary (USE: part of Sinnamary Estuary under the tidal influence but upstream of the salt intrusion) as a potential site for Hg methylation. The concentrations of all mercurial species (HgT) in the unfiltered samples (HgTUNF), in the dissolved (HgTD) and particulate (HgTP) phases of the USE average 11 ± 3, 6 ± 2 and 5 ± 3 (i.e. 600 ± 200 pmol g-1) pmol L-1 respectively. Average concentrations of monomethylmercury (MMHg) in the unfiltered (MMHgUNF), dissolved (MMHgD) and particulate (MMHgP) phases were 3.7 ± 1.0, 2.0 ± 0.9 and 1.8 ± 1.2 (i.e. 220 ± 130 pmol g-1) pmol L-1 respectively. Water oxygenation and sulfides concentrations emerged to play a critical role in controlling MMHg levels. Additionally, iron cycling, acid-base mechanisms, and edoxdependent processes were involved in the MMHg partitioning between phases. Overall, the USE constitutes a biogeochemical reactor that gathers partitioning and methylation processes. Thepermanent MMHg inputs from the anoxic freshwater end-member combined with the intense endogenous Hg methylation ensures high MMHg levels in both dissolved and particulate phases. To illustrate, the USE exports 60 ± 20 % more MMHgUNF than it imports: 5.5 ± 0.7 vs. 3.5 ± 1.2 kg y-1.

Résumé : The present work was carried out on the hydroelectric reservoir Petit-Saut on the Sinnamary river in French Guiana. Measurements were performed during the wet and dry seasons along a longitudinal gradient, from upstream of the reservoir in two inflow rivers, to the Sinnamary estuary downstream of the dam. Gold extraction has led to a marked increase in suspended matter and total mercury (HgT) in one of the rivers. Dissolved monomethylmercury (MMHg) measured in surface waters were similar for both rivers: 0.03-0.05 ng l(-1) (1.0-1.7% of the dissolved HgT). These results indicate similar methylation efficiency and/or transfer of MMHg into the dissolved fraction of the water column, independently of the amounts of inorganic mercury transported. Dissolved MMHg concentrations in surface waters of the reservoir were similar to those in the rivers, but were more than 10 times higher in deep anoxic waters, up to 0.6 ng l(-1) (20 % of dissolved HgT). The MMHg concentration profiles in the water column suggest that methylation occurs mainly in anoxic waters and sediments in relation with the activity of sulfate reducing bacteria. Dissolved MMHg concentrations measured in the Sinnamary at the base of the dam were still high (0.5 - 0.6 ng l(-1); 20 to 35% of the dissolved HgT).

Résumé : Total mercury (HgT) and monomethylmercury (MMHg) were investigated in a tropical head watershed (1 km(2)) of French Guiana. The watershed includes a pristine area on the hill slopes and a former gold mined flat in the bottomland. Concentrations of dissolved and particulate HgT and MMHg were measured in rain, throughfall, soil water and at three points along the stream. Samples were taken in-between and during 14 storm events at the beginning and middle of the 2005 and 2006 rainy seasons. Dissolved and particulate HgT concentrations in the stream slightly increased downstream, while dissolved and particulate MMHg concentrations were low at the pristine sub-watershed outlet (median = 0.006 ng L-1 and 1.84 ng g(-1), respectively) and sharply increased at the gold mined flat outlet (median = 0.056 ng L-1 and 6.80 ng g(-1), respectively). Oxisols, which are dominant in the pristine area act as a sink of HgT and MMHg from rain and throughfall inputs. Hydromorphic soils in the flat are strongly contaminated with Hg (including Hg droplets) and their structure has been disturbed by former gold-mining processes, leading to multiple stagnant water areas where biogeochemical conditions are favorable for methylation. In the former gold mined flat high dissolved MMHg concentrations (up to 0.8 ng L-1) were measured in puddles or suboxic soil pore waters, whereas high dissolved HgT concentrations were found in lower Eh conditions. Iron-reducing bacteria were suggested as the main methylators since highest concentrations for dissolved MMHg were associated with high dissolved ferrous iron concentrations. The connection between saturated areas and stagnant waters with the hydrographic network during rain events leads to the export of dissolved MMHg and HgT in stream waters, especially at the beginning of the rainy season. As both legal and illegal gold-mining continues to expand in French Guiana, an increase in dissolved and particulate MMHg emissions in the hydrographic network is expected. This will enhance MMHg bio-amplification and present a threat to local populations, whose diet relies mainly on fish.

Résumé : The distribution and speciation of mercury (Hg) in the water column, the inputs (wet deposition and tributaries) and the outputs (atmospheric evasion and outlet) of an artificial partially anoxic tropical lake (Petit-Sa-Lit reservoir, French Guiana) were investigated oil a seasonal basis in order to appraise the cycling and transformations of this metal. The total mercury (HgT) concentrations in the oxygenated epilimnetic waters averaged 5 +/- 3 pmol L-1 in the unfiltered samples (HgTUNF) and 4 +/- 2 pmol L-1 in the dissolved (HgTD) phase (< 0.45 mu m). On average, the monomethylmercury (MMHg) constituted 8%, 40%, and 18% of the HgT in the dissolved phase, tile particulate suspended matter and in tile unfiltered samples, respectively. Covariant elevated concentrations of particulate MMHg and chlorophyll a in the epilimnion suggest that phytoplankton is ail active component for the MMHg transfer in tile lake. In the anoxic hypolimnion the HgTUNF averages 13 +/- 6 pmol L-1 and tile HgTD 8 +/- 4 pmol L-1. The averages of MMHgP and MMHgD in hypolimnetic waters were two and three times the corresponding values of the epilimnion, 170 +/- 90 pmol g(-1) and 0.9 +/- 0.5 pmol L-1, respectively. In the long dry and wet seasons, at the flooded forest and upstream dam sampling stations, the vertical profiles of MMHgD concentrations accounted for two distinct maxima: one just below the oxycline and the other near the benthic interface. Direct wet atmospheric deposition accounted for 14 moles yr(-1) HgTUNF, with 0.7 moles yr(-1) as MMHgUNF, while circa 76 moles yr(-1) of HgTUNF, with 4.7 moles yr(-1) as MMHgUNF, coming from tributaries. Circa 78 moles (similar to 17% as MMHg) are annually exported through tile dam, while 23 moles yr(-1) of Hg-0 evolve in the atmosphere. A mass balance calculation suggests that the endogenic production of MMHgUNF attained 8.1 moles yr(-1), corresponding to a methylation rate of 0.06% d(-1). As a result, tile Petit-Saut reservoir is a large man-made reactor that has extensively altered mercury speciation in favor of methylated species.

Résumé : Climatic and anthropogenic changes are able to engender modifications in the aerosol composition at different geographical and temporal scales The present study addresses this question for the trace metal concentrations (TM = Al, Fe, Mn, Co, Ni, Cu, Pb, Cd and Zn) of aerosol from the North-western Mediterranean coast of France (Cap Ferrat, nearby Nice) between 1986 and 2008. From seasonal variations (2006-08) and decadal trends (1986-2008) of TM concentrations. three groups of elements can be distinguished They consist of different aerosol sources: crustal-derived elements (Al, Fe. Mn and Co), trace metals of anthropogenic origin (Pb, Cd and Zn) and a third, intermediate, group of trace metals that presented both anthropogenic and natural/crustal influences (Ni and Cu). Reproducible seasonal patterns were observed for crustal and intermediate elements with highest concentrations between May and November. while anthropogenic trace metals did not show a pronounced seasonal cycle Nevertheless, highest concentrations of anthropogenic trace metals occurred mostly in autumn/winter. Aerosol concentrations of anthropogenic TMs decreased remarkably over the last two decades, while crustal trace metals did not show any evolution Nickel and copper aerosol concentrations remained constant, as well. Lead concentrations decreased from 1986 (29.34 ng m(-3)) to 2008 (3.33 ng m(-3)). overall by 90%. Cadmium and zinc aerosol concentrations decreased by 66 and 54%, respectively, between 1998 and 2006-08. from 0.27 to 0.09 ng m(-3) and from 23.9 to 10.9 ng m(-3). respectively.
