Malaria remains one of the most common vector-borne diseases in the world and the definition of novel control strategies can benefit from the modeling of transmission processes. However, data-driven models are often difficult to build, as data are very often incomplete, heterogeneous in nature and in quality, and/or biased. In this context, a knowledge-based approach is proposed to build a robust and general landscape-based hazard index for malaria transmission that is tailored to the Amazonian region. A partial knowledge-based model of the risk of malaria transmission in the Amazonian region, based on landscape features and extracted from a systematic literature review, was used. Spatialization of the model was obtained by generating land use and land cover maps of the cross-border area between French Guiana and Brazil, followed by computing and combining landscape metrics to build a set of normalized landscape-based hazard indices. An empirical selection of the best index was performed by comparing the indices in terms of adequacy with the knowledge-based model, intelligibility and correlation with P. falciparum incidence rates. The selected index is easy to interpret and successfully represents the current knowledge about the role played by landscape patterns in malaria transmission within the study area. It was significantly associated with P. falciparum incidence rates, using the Pearson and Spearman correlation coefficients (up to 0.79 and 0.75, respectively; p-value < 0.001), and the linear regression coefficient of determination (reaching 0.63; p-values < 0.001). This study establishes a spatial knowledge-driven, landscape-based hazard malaria index using remote sensing that can be easily produced on a regular basis and might be useful for malaria prediction, surveillance, and control. Malaria is a major health problem for people who live on the border between Brazil and French Guiana. Here we discuss Plasmodium vivax distribution pattern in the town of Oiapoque, Amapá State using the circumsporozoite (CS) gene as a marker. Ninety-one peripheral blood samples from P. vivax patients have been studied. Of these, 64 individuals were from the municipality of Oiapoque (Amapá State, Brazil) and 27 patients from French Guiana (August to December 2011). DNA extraction was performed, and a fragment of the P. vivax CS gene was subsequently analyzed using PCR/RFLP. The VK210 genotype was the most common in both countries (48.36% in Brazil and 14.28% in French Guiana), followed by the P. vivax-like (1.10% in both Brazil and French Guiana) and VK247 (1.10% only in Brazil) in single infections. We were able to detect all three CS genotypes simultaneously in mixed infections. There were no statistically significant differences either regarding infection site or parasitaemia among individuals with different genotypes. These results suggest that the same genotypes circulating in French Guiana are found in the municipality of Oiapoque in Brazil. These findings suggest that there may be a dispersion of parasitic populations occurring between the two countries. Most likely, this distribution is associated with prolonged and/or more complex transmission patterns of these genotypes in Brazil, bordering French Guiana.

BACKGROUND:In French Guiana, a French overseas territory in South America, 6 to 10 thousands undocumented persons work illegally in gold mining sites in the Amazonian forest. Precarious life conditions lead to poor health but few data exist on the health status of illegal gold miners in French Guiana. The objective of this article was to describe the sociodemographic and health status of this vulnerable population.METHOD:A prospective cross-sectional survey was conducted in 2015 on gold mine supply sites at the border between French Guiana and Suriname. Health status was assessed through medical examination, past medical history, haemoglobin concentration, and HIV and malaria testing. A questionnaire was used to collect data about the migration itinerary and life conditions on mining sites.RESULTS:Among the 421 adults included in the study, 93.8% (395/421) were Brazilian, mainly from Maranhão (55.7%, 220/395), the poorest Brazilian state. The sex ratio was 2.4. Overall, 48% of persons never went to school or beyond the primary level. The median time spent in gold mining was quite long (10 years), with a high turn-over. One third of the surveyed population (37.1%, 156/421) had high blood pressure, and only two had a medical follow-up. Most persons had experienced malaria (89.3%, 376/421). They declared frequent arboviroses and digestive disorders. Active leishmaniasis was observed in 8.3% of gold miners. Among women, 28.5% were anemic. Concerning HIV, 36.6% (154/421) of persons, mainly men, never got tested before and 6 were tested positive, which represented an HIV prevalence of 1.43% (95%CI =0.29-2.5).CONCLUSION:These findings support the hypothesis that mining in remote areas is linked to several specific illnesses. Theoretically, gold miners would be presumed to start their economical migration to French Guiana as a healthy group. However, their strenuous working and living conditions there lead to poor health caused by infectious and non infectious diseases. This description of their health status is precious for health policy planners in French Guiana given the importance of controlling communicable disease, and the severity and range of specific illnesses acquired by this neglected population.TRIAL REGISTRATION:Clinical trial registration PRS N° NCT02903706 . Plasmodium vivax malaria is an important public health issue in the Amazon region, and it accounts for approximately 84 % of cases of the disease. Migration across the border between Brazil and French Guiana contributes to the maintenance of the disease. The aim of this study was to evaluate the therapeutic and parasitological responses of patients with P. vivax malaria treated with chloroquine and primaquine in the socio-environmental context of cross-border interactions between Brazil and French Guiana. The factors controlled were diagnostic agreement, adherence, adjustment of primaquine doses for patient weight, and quality of the drugs used. A prospective study was conducted in 2011 with 103 individuals aged 10-60 years with a positive diagnosis of P. vivax treated with chloroquine (10 mg base/kg on the first day, followed by 7.5 mg/kg on the second and third days) and primaquine for 7 days, who were followed for 28 days. The primaquine doses were adjusted for the patients' weight. A number of factors were determined: epidemiological characteristics, origin of patients, signs and symptoms, initial parasitaemia and parasitaemia clearance time, blood concentrations of chloroquine and primaquine, quality of anti-malarial drugs and diagnostic agreement. Ninety-five patients were followed for 28 days. There was a 100 % agreement in microscopic diagnosis between field laboratory and reference centre. The adhesion to the treatment was 100 %. Of these patients, 32.6 % received a weight-adjusted dose of primaquine. The chloroquine and primaquine tablets were consistent with the optimal quality limits for human consumption. The investigated patients achieved optimal blood exposure to anti-malarial drugs. The parasitological and therapeutic response was adequate in 99.0 % of cases. In the municipality of Oiapoque, the therapeutic regime used for the treatment of P. vivax malaria using chloroquine combined with primaquine remains effective, when external factors are controlled, such as the quality of anti-malarial drugs, the adhesion to the treatment prescribed, the correct diagnostic and the adjustment of primaquine dose for patient body weight.

Résumé : Le Brésil et la France partagent une frontière commune sur plus de 700 kilomètres, via la Guyane, région monodépartementale d’Outre-Mer française, et l’Amapá, Etat de la République fédérative du Brésil. Réalité socio-spatiale en marges à toutes les échelles malgré le différentiel socio-économique fort qu’elle marque, la frontière franco-brésilienne est devenue, depuis les années 1990, une interface politique, support de la relance des relations internationales franco-brésiliennes et des relations entre Guyane et Amapá.La thèse pose la question de cet écart en se concentrant sur la partie estuarienne de la frontière fluviale de l’Oyapock, autour des bourgs riverains de Saint-Georges (Guyane française) et d’Oiapoque (Brésil), entre lesquels un pont binational d’envergure est en construction, malgré leur rattachement routier à la marge aux polarités régionales de Cayenne et de Macapá.
Brazil and France share a common border of over 700 km via French Guiana, a French Overseas monodepartmental region, and Amapá, State of the Federal Republic of Brazil. Socio-spatial margin at all scales despite the strong socio-economic differential it marks, the Franco-Brazilian border has become since the 1990s a political interface, support for the revival of the Franco-Brazilian international relations and of the relations between French Guiana and Amapá. The thesis raises the question of this gap, focusing on the estuarine portion of the border river of the Oyapock, around the towns of Saint-Georges (French Guiana) and Oiapoque (Brazil) ; a significant bi-national bridge is under construction between these two towns, despite their road connection as margins to the regional polarities of Cayenne and Macapá.
Brasil e França compartilham uma fronteira comum com mais de 700 km através da Guiana Francesa, Região monodepartamental ultramarina francesa, e do Amapá, Estado da República Federativa do Brasil. Realidade sócio-espacial à margem a todas as escalas, apesar do forte diferencial sócio-econômico que a marca, a fronteira franco-brasileira tornou-se, desde os anos 90, uma interface política, suporte para a retomada das relações internacionais franco-brasileiras e das relações entre a Guiana Francesa e o Amapá. A tese expõe a questão deste desnível, centrando-se na porção estuarina da fronteira fluvial do Oiapoque, ao redor das pequenas cidades de Saint-Georges (Guiana Francesa) e de Oiapoque (Brasil), entre as quais uma ponte binacional de grande porte está em construção, apesar da ligação rodoviária delas como margens aos pólos regionais de Caiena e Macapá.

BackgroundMalaria is endemic in French Guiana, an overseas territory of France on the Guiana Shield. Since 2005, notified malaria cases are decreasing. However, new data show that malaria affects many Brazilian gold miners working illegally in French Guiana, the majority of whom are not counted in official data. In addition, one major concern is the usual practice of improper self-treatment in this mining population, raising fear of the development of antimalarial resistance. This prospective study, conducted in 2015, aimed to estimate the prevalence of Plasmodium spp. in illegal gold miners working in French Guiana. MethodsThe recruitment of gold miners was carried out in resting sites along the French Guiana-Suriname border, where they go for supplies, medical care or leisure. After recording agreement, three malaria diagnostic methods were performed: rapid diagnostic test, microscopy and PCR,ResultsAmong 421 persons recruited in the study, malaria prevalence, detected by nested-PCR, was 22.3% (CI=[18.3-26.3], n=94/421) of which 84% were asymptomatic. ConclusionThis significant malaria reservoir in a mobile and illegal population with difficult access to a health care system raises the threat of artemisinin resistance and puts the population of the Guiana Shield at risk of new transmission foci while countries of the region aim at malaria elimination. Even though French legislation may hamper dealing with this population, France must face the reality of malaria in illegal gold miners in order to meet its commitment to malaria elimination.

Résumé : Les espaces naturels protégés sont aujourd’hui à l‘échelle mondiale l’une des principales politiques publiques mises en œuvre pour faire face à la destruction des écosystèmes. Leur nombre a beaucoup augmenté ces dernières années et les modes d’action qu’ils recouvrent se sont considérablement diversifiés. Pour autant, les écosystèmes continuent à se dégrader, et l’efficacité de ces dispositifs est souvent remise en cause. Cette thèse vise à analyser les facteurs influant sur l’efficacité des espaces protégés pour la conservation d’écosystèmes amazoniens encore peu dégradés. Elle étudie pour cela la mise en œuvre de dispositifs relevant de doctrines de gestion de l’environnement différentes, basées sur (i) la limitation réglementaire maximale des activités humaines impactantes pour les écosystèmes, (ii) la gestion des ressources par des populations locales ou traditionnelles ou (iii) la mise en place d’une gestion forestière durable sur des terres publiques. Nous nous plaçons dans une posture comparative entre la Guyane (France) et l’Amapá (Brésil), territoires partageant une frontière commune, principalement matérialisée par le fleuve Oyapock. La comparaison internationale entre ces deux régions présentant une couverture exceptionnelle en espaces protégés, dans des conditions écologiques et géographiques relativement similaires, nous permet d’observer l’influence du contexte historique et sociopolitique sur les modes de prise en charge de la gestion de l’environnement par les aires protégées. Nous nous sommes basés sur une approche constructiviste, appuyée sur une production de données principalement qualitatives (entretiens semi-directifs, analyse de documents, observation participante…). Nous avons ainsi procédé à une déconstruction critique des dispositifs « aires protégées », permettant de mettre en lisibilité les enjeux environnementaux qu’ils portent, et d’analyser l’efficacité environnementale des stratégies mises en œuvre. Cette analyse s’est articulée autour d’une lecture à la fois diachronique et multiscalaire des processus de gestion.Nous montrons que la mise en place des espaces protégés de Guyane et d’Amapá a été portée par des coalitions d’acteurs structurées autour de doctrines de gestion, c’est à dire de conceptions partagées des conditions de mise en œuvre d’une « bonne gestion environnementale ». Les dispositifs créés ont hérité des ressources stratégiques d’action et de la légitimité de ces coalitions. Les coalitions porteuses de la création des espaces protégés en Amapá sont fortement articulées aux mouvements sociaux et environnementaux plus généraux de l’Amazonie brésilienne, notamment pour les revendications socio-environnementales émergeant à la sortie de la période dictatoriale. En Guyane française, les espaces naturels protégés sont principalement la résultante de compromis entre d’une part une volonté d’exemplarité de l’action de la France en Amazonie, et d’autre part une recherche de minimisation des conflits avec les acteurs politiques locaux.
Nowadays, natural protected areas are one of the main public policies implemented at the international level in order to prevent the destruction of ecosystems. During the last decades, they became very numerous, and mobilized more and more diversified modes of action. However, natural ecosystems continue to deteriorate, and the effectiveness of protected areas is often questioned. The aim of this thesis was to analyse the factors influencing the effectiveness of protected areas in a context of Amazonian ecosystems that are still well preserved. We observed the implementation of environmental management arrangement (protected areas) that are based on various doctrines: (i) the strong limitation of all harmful human activities through legislation; (ii) the management of resources by local or traditional populations; and (iii) the implementation of sustainable forest management on public lands. We adopted a comparative approach between Guyana (France) and Amapá (Brazil), territories that share a common border, mainly identifiable by the Oyapock River. These two territories present quite similar ecological and geographical conditions, and both have an exceptional coverage of protected areas. Therefore this international comparison allowed us to observe the influence of the historical and socio-political context on the implementation of protected areas. We used a constructivist approach, mainly based on qualitative data (semi-structured interviews, document analysis, participant observation...). We effected a critical deconstruction of "protected areas" management arrangements in order to make their environmental objectives intelligible, and we analysed the environmental effectiveness of the strategies implemented. This analysis was based on a diachronic and multiscalar lecture of management processes. We show that the establishment of protected areas in Guiana and Amapá was led by coalitions of actors structured around management doctrines, i.e. shared conceptions of "good environmental management". Management arrangements created inherited the strategic resources of action and the legitimacy of these coalitions. In Amapá, coalitions were strongly articulated with wider social and environmental movements of the Brazilian Amazon, especially in the case of socio-environmental claims rising at the end of the dictatorship. In French Guiana, the natural protected areas are mainly the result of compromise between the French willingness to serve as an example in the Amazon region, and the minimisation of conflicts with local politicians.
