Montego Bay, Saint James, Jamaica became popular as a sea bathing destination when in 1906 Doctor Alexander James McCatty donated the land to the community and rumors spread that the waters off this beach had health benefits. The beach received its name from Doctor McCatty and his physician friends and because the original entrance was through a cave, which was destroyed in a 1932 hurricane. The image is of tourists sea bathing in the Caribbean Sea while children play in the sand of Doctor's Cave Beach.

The Texaco Astronauts steel band, popular during the 1960s, were sponsored by Texaco, an oil company. In the distance are pedestrians on a nearby street. In the garden, near the practicing band, are empty tables and chairs on a gravel path. Mango, palm, and various trees grow in the garden.

Résumé : Contract laborers were imported from China to Cuba in the nineteenth century to alleviate the severe labor shortage caused by the expansion of the sugar estates. Between 1847 and 1878, approximately 145,000 Chinese arrived in the island. As they were expected to work under conditions similar to slavery, the death rate was high. By 1899, the population was reduced to 14,863. The artist, Frederic Remington, well-known for his portrayal of the Old West, began his career at Harper's Weekly. On assignment as artist and correspondent in Cuba during the 1890s, he reported on conditions on the island, as well as the Spanish American War in 1898.

He was born at Wormwood Gate, Dublin to Edward Madden, a silk manufacturer. He was educated at private schools. He studied medicine in Paris, Italy, and St George's Hospital, London. While in Naples he became acquainted with Lady Blessington and her circle.[1] Madden was employed in the British civil service from 1833, first as a justice of the peace in Jamaica, where he was one of six Special Magistrates sent to oversee the eventual liberation of Jamaica's slave population, according to the terms of the Slavery Abolition Act 1833. From 1835 he was Superintendent of the freed Africans in Havana. His son, Thomas More Madden, who later became a surgeon and writer, was born there. In 1839 he became the investigating officer into the slave trade on the west coast of Africa, in 1847 the secretary for the West Australian colonies. He returned to Dublin and in 1850 he was named secretary of the Office for Loan Funds in Dublin.[2] He died at his home in Booterstown, just south of Dublin city, in 1886 and is interred in Donnybrook Cemetery.

Prostate cancer disparities have been reported in men of African descent who show the highest incidence, mortality, compared with other ethnic groups. Few studies have explored the genetic and environmental factors for prostate cancer in men of African ancestry. The glutathione-S-transferases family conjugates carcinogens before their excretion and is expressed in prostate tissue. This study addressed the role of GSTM1 and GSTT1 deletions on prostate cancer risk in populations of African descent. This multi-institutional case-control study gathered data from the Genetic Susceptibility to Environmental Carcinogens (GSEC) database, the African-Caribbean Cancer Consortium (AC3) and Men of African Descent and Carcinoma of the Prostate Consortium (MADCaP). The analysis included 10 studies (1715 cases and 2363 controls), five in African-Americans, three in African-Caribbean and two in African men. Both the GSTM1 and the GSTT1 deletions showed significant inverse associations with prostate cancer [odds ratio (OR): 0.90, 95% confidence interval (CI) 0.83-0.97 and OR 0.88, 95% CI: 0.82-0.96, respectively]. The association was restricted to Caribbean and African populations. A significant positive association was observed between GSTM1 deletion and prostate cancer in smokers in African-American studies (OR: 1.28, 95% CI: 1.01-1.56), whereas a reduced risk was observed in never-smokers (OR: 0.66, 95% CI: 0.46-0.95). The risk of prostate cancer increased across quartiles of pack-years among subjects carrying the deletion of GSTM1 but not among subjects carrying a functional GSTM1. Gene-environment interaction between smoking and GSTM1 may be involved in the etiology of prostate cancer in populations of African descent.

OBJECTIVE: Several studies demonstrate that the perception of overweight is associated with attempts at weight control in overweight subjects. Our objective is to describe the factors associated with the perception of overweight in an adult Caribbean population, and in particular, to assess the relationship between the perception of overweight and previous medical diagnosis of overweight. METHOD AND DESIGN: Consant was a cross-sectional study carried out in 2007 on a representative sample of the adult Guadeloupean population. Included were 1,005 subjects aged 25-74 years, drawn from stratified random sampling, and interviewed at home. The perception of overweight was evaluated by the question: "Do you consider yourself to be overweight?" with a binary response. RESULTS: Among overweight subjects (Body Mass Index ≥ 25 kg/m2, n = 576), 38.6% provided an incorrect assessment of their weight status. This proportion falls to 17.4% in those who stated to have previously received a medical diagnosis of overweight. However, only 48.8% of overweight subjects stated to have received such a diagnosis. A logistic regression shows that the underestimation of weight status is associated with the absence of previous diagnosis for overweight (OR: 5.35 - p < 10-3), as well as an age greater than or equal to 55 years (OR: 2.91 - p < 10-3), a BMI of less than 30 kg/m² (OR: 2.26 - p: 0.001), the male sex (OR: 1.86 - p: 0.004), and a low income level (OR: 1.62 - p: 0.030), but not a lack of awareness regarding the role of weight control in the prevention of cardiovascular diseases. Similar results are observed if waist circumference is taken as a factor instead of BMI. CONCLUSION: In this population, the lack of medical diagnosis of overweight was the leading factor associated with misperception of overweight. Screening for overweight therefore appears to be one of the preconditions to weight management, and remains a fundamental but often unused element in the fight against obesity.

Purpose: Active surveillance is a treatment option for favorable risk prostate cancer. However, data are missing on populations of African descent. We evaluated the safety and benefit of active surveillance in an African Caribbean cohort with favorable risk prostate cancer. Materials and Methods: Between 2005 and 2016, a single center, prospective cohort study was performed in Guadeloupe, French West Indies, including patients on active surveillance who had low risk prostate cancer (prostate specific antigen 10 ng/ml or less and Gleason score 6 or less) or favorable intermediate risk prostate cancer (prostate specific antigen 10 to 20 ng/ml, Gleason score 3 + 4 or less and life expectancy less than 10 years). Treatment was recommended in case of grade progression, increased tumor volume, prostate cancer doubling time less than 36 months or patient wish. Overall survival, disease specific survival and duration of active surveillance were calculated with the KaplanMeier method. Multivariate analysis was performed using the Cox proportional hazards model to identify predictors of active surveillance termination. Results: A total of 234 patients with a median age of 64 years were enrolled in study. Median followup was 4 years (IQR 2.3-5.5). Overall survival at 30 months, 5 years and 10 years was 99.5%, 98.5% and 90.7%, respectively. Disease specific survival at 30 months, and 5 and 10 years was 100%. At 30 months, 5 years and 10 years 72.7%, 52.6% and 40.4% of patients, respectively, remained untreated and on active surveillance. Age (HR 0.96 per additional year, 95% CI 0.93-0.99) and prostate specific antigen density (HR 1.52 per additional 0.1 ng/ml, 95% CI 1.20-1.89) were found to be independent predictors of active surveillance termination. Conclusions: Active surveillance is safe and beneficial for highly selected African Caribbean patients. It seems to be feasible for patients at low risk and intermediate favorable risk. Prostate specific antigen density could help better select these patients.

Steel pan music is most often associated with Trinidad and Tobago, where the art form originated, but the music is also popular in Barbados. Steel pans are made from oil drums that have had the drum head shaped with slopes and slants to make music note sounds. These drums have been painted blue and yellow to reflect the Barbados flag colors. This steel pan band includes a musician using shak-shaks (similar to maracas), which is a hollow rattle used in the Lesser Antilles. Cockspur Fine Rum box, pictured, has been produced in Barbados since 1884, when Danish seaman Valdemar Hanschell created the rum, which is distilled from fermented molasses. The cruise terminal, pictured, would contain departments to manage cruise passengers, such as Immigration and Customs. Slide labeled Barb 8.

Cancer is the main complication of transplantation surgery. The literature concerning renal transplant recipients among the Afro-Caribbean population is scant. The aim of this study was to determine the incidence of cancer in these patients, with the secondary objective being to identify predisposing factors for cancer. This was an epidemiological and retrospective study that included all Guadeloupians of phototype V-VI undergoing renal transplantation from 01/01/2004 to 31/12/2011. Skin cancer screening was performed before transplantation and during an annual dermatological consultation following transplantation. Screening for non-cutaneous cancers was guided by clinical symptoms or by the results of the screening examinations recommended in the current guidelines. At the study time-point (31/12/2011), all patients were examined by a dermatologist. One hundred and two patients were included : 42 women and 60 men (mean age: 52.1±11.6 years at transplantation). Eight cancers were diagnosed. The cumulative incidence of cancer was 7.8% at 3 years. Three factors were associated with more rapid onset of cancer: personal history or familial history of cancer, and genital lesion induced by HPV. Our results suggest a low incidence of cancer in Afro-Caribbean renal transplant patients. Personal or family history of cancer and HPV-induced genital lesions would appear to accelerate the onset of cancer in this population.

BACKGROUND: Recent studies suggest that a Mediterranean dietary pattern during pregnancy may influence pregnancy outcomes. The aim of this study was to evaluate the effect of adherence to a Mediterranean diet (MD) during pregnancy on fetal growth restriction (FGR) and preterm delivery (PTD) in a French Caribbean island where the population is largely of African descent and presents dietary patterns similar to MD. METHODS: Using data from the TIMOUN Mother-Child Cohort Study conducted in Guadeloupe (French West Indies) between 2004 and 2007, we analysed data for 728 pregnant women who delivered liveborn singletons without any major congenital malformations. Degree of adherence to MD during pregnancy was evaluated with a semi-quantitative food frequency questionnaire based on nine dietary criteria. Multiple logistic regression models were used to analyse birth outcomes while taking potential confounders into account. RESULTS: Overall there was no association between MD adherence during pregnancy and the risk of PTD or FGR. However, pre-pregnancy body mass index was a strong effect modifier, and MD adherence was associated with a decreased risk of PTD specifically in overweight and obese women (adjusted odds ratio 0.7, 95% confidence interval 0.6, 0.9) (P heterogeneity <0.01). CONCLUSIONS: These results suggest that Caribbean diet during pregnancy may carry some benefits of MD and may contribute to reduce the risk of PTD in overweight and obese pregnant women.

TRAMIL is a research program applied to the use of medicinal plants in the Caribbean, established in the 1980s. Significant uses of herbal remedies to address self limiting health problems are identified through participatory ethnopharmacological surveys and are the subject of scientific validation. The results are then disseminated to several audiences.While comprising an international scientific contribution to academic research, the TRAMIL network activities value the popular and local knowledge. This fertile relationship between local knowledge and scientific research draws answers to key challenges for Caribbean societies: improving the health of Caribbean populations first and foremost, but also indirectly developing economic prospects in various sectors (agriculture, industry, tourism, and health).The success of TRAMIL is based on a rigorous methodology and on a noteworthy informal network of collaborators invested at different levels of the program, locally and / or regionally. TRAMIL is one of the few trilingual networks that have been active for nearly forty years, well before the popularization of the Internet, in more than twenty Caribbean territories. It involves people and communities, health care professionals, researchers in many disciplines, and information and documentation professionals.This paper proposes to explore these new ways of working together and the accomplishments to which they give rise by presenting some concrete examples of the TRAMIL program in Puerto Rico and examining the program's website and the complex challenges faced by the stakeholders involved in its implementation.
TRAMIL es un programa de investigación aplicada a la medicina popular del Caribe, iniciado en la década de los ‘80. Su trabajo se basa en llevar a cabo encuestas etnofarmacológicas participativas para identificar usos significativos de remedios de plantas, que son objeto de la validación científica, para tratar problemas comunes de salud. La difusión de los resultados de validación de los remedios con uso significativo completa las líneas de acción de TRAMIL. Al tiempo que representan una contribución científica internacional para la investigación académica, las actividades de TRAMIL constituyen la valoración de los saberes populares y locales. Esta relación fecunda entre el conocimiento local y la investigación científica es una respuesta a desafíos fundamentales para las sociedades del Caribe: mejorar la salud de las poblaciones del Caribe directa y en primer lugar, pero también, indirectamente, desarrollar oportunidades económicas en varios sectores (agricultura, la industria, el turismo y la salud).El éxito de TRAMIL se basa en una metodología rigurosa y una importante red informal de colaboradores que participan en diversas funciones del programa, a nivel de país y regional. TRAMIL es una de las pocas redes totalmente trilingües, activa durante casi cuarenta años, mucho antes de la popularización del Internet, en más de una veintena de territorio caribeños. Asocia especialmente a comunidades, profesionales de la salud, investigadores en muchas disciplinas, y profesionales de la información y documentación.Este trabajo tiene como objetivos explorar las modalidades de cooperación y logros sin precedentes de TRAMIL, presentando algunos ejemplos concretos del programa en Puerto Rico y examinar el sitio Web del programa y la complejidad de los desafíos que han enfrentado los actores involucrados en su implementación.

Les usages significatifs des plantes pour répondre à des problèmes de santé sont recensés par des enquêtes ethnopharmacologiques participatives puis font l’objet de travaux scientifiques destinés à les valider. Les résultats sont ensuite diffusés auprès de larges publics.Tout en constituant un apport scientifique international à la recherche universitaire, les activités du réseau TRAMIL s’inscrivent dans la valorisation des savoirs locaux. Cette relation fertile entre savoirs locaux et recherche scientifique dessine des réponses à des défis essentiels pour les sociétés caribéennes : l’amélioration de la santé des populations caribéennes de façon directe et en premier lieu, mais également, de façon indirecte, le développement de perspectives économiques dans des secteurs divers (agriculture, industrie, tourisme, santé).Le succès de TRAMIL repose sur une méthodologie rigoureuse et sur un important réseau informel d’acteurs investis à différents niveaux du programme, sur le plan local et/ou pancaribéen. C’est l’un des rares réseaux entièrement trilingue, actifs depuis près de quarante ans (bien avant la popularisation d’internet), dans plus d’une vingtaine de territoire de la Caraïbe. Il associe notamment les populations, les professionnels de santé, les chercheurs dans de nombreuses disciplines, et les professionnels de l’information et de la documentation.La présente contribution propose d’explorer ces modalités de collaboration inédites et les réalisations auxquelles elles donnent lieu, en présentant quelques exemples concrets du programme TRAMIL à Porto Rico, puis en examinant le site internet du programme et les défis complexes auxquels il a confronté les acteurs impliqués dans sa réalisation.

In French Guiana, severe cases of toxoplasmosis in immunocompetent patients are associated with atypical strains of Toxoplasma gondii linked to a wild neotropical rainforest cycle and a higher genetic diversity than usually observed for T. gondii isolates from anthropized environment. This raises the question of the impact of anthropization of the natural environment, on genetic diversity and on the population structure of T. gondii. However, few data are available on strains circulating in the anthropized areas from French Guiana. Seropositive animals originating mainly from anthropized sub-urban areas and punctually from wild environment in French Guiana were analyzed for T. gondii isolation and genotyping. Thirty-three strains were obtained by bioassay in mice and compared with 18 previously reported isolates chiefly originating from the Amazon rainforest. The genotyping analysis performed with 15 microsatellite markers located on 12 different chromosomes revealed a lower genetic diversity in the anthropized environment. Results were analyzed in terms of population structure by clustering methods, Neighbor-joining trees reconstruction based on genetic distances, F(ST,) Mantel's tests and linkage disequilibrium. They clearly showed a genetic differentiation between strains associated to the anthropized environment and those associated to the wild, but with some inbreeding between them. The majority of strains from the anthropized environment were clustered into additional lineages of T. gondii that are common in the Caribbean. In conclusion the two environmental populations "wild" and "anthropized" were genetically well differentiated. The anthropization of the environment seems to be accompanied with a decreased diversity of T. gondii associated with a greater structure of the populations. We detected potential interpenetration and genetic exchanges between these two environmental populations. As a higher pathogenicity in human of "wild" genotypes has been described, the interpenetration of both environments leads to hybridization between strains that may be at risk for human health.
