29 inorganic compounds (Al, As, B, Ba, Be, Bi, Ca, Cd, Ce, Co, Cr, Cu, Fe, Gd, K, Mg, Mn, Mo, Na, Nd, Ni, Pb, Sb, Se, Sr, Ti, U, V and Zn) were measured in the tap water of 484 representative homes of children aged 6 months to 6 years in metropolitan France in 2008-2009. Parents were asked whether their children consumed tap water. Sampling design and sampling weights were taken into account to estimate element concentrations in tap water supplied to the 3,581,991 homes of 4,923,058 children aged 6 months to 6 years. Median and 95th percentiles of concentrations in tap water were in mu g/L: Al: <10, 483, As: 02, 2.1; B: <100, 100; Ba: 30.7, 149.4; Ca: 85,000, 121,700; Cd: <0.5, <0.5; Ce: <0.5, <0.5; Co: <0.5, 0.8; Cr: <5, <5; Cu: 70, 720; K: 2210, 6740; Fe: <20, 46; Mn: <5, <5; Mo: <0.5,1.5; Na: 14,500, 66,800; Ni: <2,10.2; Mg: 6500,21,200; Pb: <1, 5.4; Sb: <0.5, <0.5; Se: <1, 6.7; Sr: 256.9, 1004; TI: <0.5, <0.5; U: <0.5, 2.4; V: <1, 1; Zn; 53, 208. Of the 2,977,123 young children drinking tap water in France, some were drinking water having concentrations above the 2011 World Health Organization drinking water quality guidelines: respectively 498 (CI 95%: 0-1484) over 700 mu g/L of Ba; 121,581 (CI 95%: 7091-236,070) over 50 mg/L of Na; 2044 (CI 95%: 0-6132) over 70 mu g/L of Ni, and 78,466 (17,171-139,761) over 10 mu g/L of Pb. Since it is representative, this tap water contamination data can be used for integrated exposure assessment, in conjunction with diet and environmental (dust and soil) exposure data. Children are exposed to toxic metals and metalloids via their diet and environment. Our objective was to assess the aggregate chronic exposure of children aged 3–6 years, living in France, to As, Cd, Cr, Cu, Mn, Pb, Sb, Sr, and V present in diet, tap water, air, soil and floor dust in the years 2007–2009. Dietary data came from the French Total Diet Study, while concentrations in residential tap water, soil and indoor floor dust came from the ‘Plomb-Habitat’ nationwide representative survey on children's lead exposure at home. Indoor air concentrations were assumed to be equal to outdoor air concentrations, which were retrieved from regulatory measurements networks. Human exposure factors were retrieved from literature. Data were combined with Monte Carlo simulations. Median exposures were 1.7, 0.3, 10.2, 34.1, 60.3, 0.7, 0.1, 44.3, 1.5 and 95th percentiles were 4.4, 0.5, 15.8, 61.3, 98.3, 2.5, 0.1, 111.1, 2.9 μg/kg bw/d for As, Cd, Cr, Cu, Mn, Pb, Sb, Sr, and V respectively. Dietary exposures dominate aggregate exposures, with the notable exception of Pb - for which soils and indoor floor dust ingestion contribute most at the 95th percentile. The strengths of this study are that it aggregates exposures that are often estimated separately, and uses a large amount of representative data. This assessment is limited to main diet and residential exposure, and does not take into account the relative bioavailability of compounds. These results could be used to help target prevention strategies.

Background: Blood Lead Levels (BLLs) have substantially decreased in recent decades in children in France. However, further reducing exposure is a public health goal because there is no clear toxicological threshold. The identification of the environmental determinants of BLLs as well as risk factors associated with high BLLs is important to update prevention strategies. We aimed to estimate the contribution of environmental sources of lead to different BLLs in children in France.Methods: We enrolled 484 children aged from 6 months to 6 years, in a nationwide cross-sectional survey in 2008-2009. We measured lead concentrations in blood and environmental samples (water, soils, household settled dusts, paints, cosmetics and traditional cookware). We performed two models: a multivariate generalized additive model on the geometric mean (GM), and a quantile regression model on the 10th, 25th, 50th, 75th and 90th quantile of BLLs. Results: The GM of BLLs was 13.8 μg/L (=1.38 µg/dL) (95% Confidence Intervals (CI): 12.7-14.9) and the 90th quantile was 25.7 μg/L (CI: 24.2-29.5). Household and common area dust, tap water, interior paint, ceramic cookware, traditional cosmetics, playground soil and dust, and environmental tobacco smoke were associated with the GM of BLLs. Household dust and tap water made the largest contributions to both the GM and the 90th quantile of BLLs. The concentration of lead in dust was positively correlated with all quantiles of BLLs even at low concentrations. Lead concentrations in tap water above 5 µg/L were also positively correlated with the GM, 75th and 90th quantiles of BLLs in children drinking tap water.Conclusions: Preventative actions must target household settled dust and tap water to reduce the BLLs of children in France. The use of traditional cosmetics should be avoided whereas ceramic cookware should be limited to decorative purposes.

Résumé : Background. Chlorine is the most widely used disinfectant for drinking water treatment. During treatment process, the natural organic matter present in water reacts with chlorine to form disinfection by-products (DBPs), such as trihalomethanes (THMs) and haloacetic acids (HAAs), the most occurring ones. Several identified DBPs were recognized carcinogens or fetotoxic in animals. We conducted two epidemiological studies to assess the health effects of human exposure to DBPs regarding cancer and reproduction. Association between THM exposure and the risk of bladder cancer: a pooled analysis of 3 European case-control studies. The study included 2381 cases and 3086 controls from 3 case-control studies (France, Finland, Spain). Environmental exposure to SPCs was measured by THM concentrations in water distribution systems, retrospectively estimated through the 40-year exposure window. Information about water uses (tap water ingestion, showers and baths, swimming-pool attendance) was available. An increased risk of bladder cancer was observed, in men only, with increasing THM concentrations in water systems and duration of exposure to chlorinated surface water. No association was observed with exposure through tap water ingestion. Exposure through showers, baths and swimming in pools (available in Spanish study only) was associated with an increased risk of bladder cancer. Three North-American case-control studies were joined to the three European ones into a meta-regression. No specific dose-response relationship was identified for European studies. A global dose-response relationship was consequently estimated, including 4351 cases and 7055 controls. Association between DBP exposure and the risk of preterm birth and Intra-Uterine Growth Retardation (IUGR). This study was conducted within the PELAGIE cohort (3400 pregnant women included in early pregnancy, in Brittany, France, between 2002 and 2006). Exposure during pregnancy was measured by 2 indicators: THM concentrations in water distribution systems and a biomarker of HAA ingestion, the level of trichloroacetic acid in maternal urine at inclusion (in a nested case-control study). The regulatory database SISE-Eaux was used to estimate THMs concentrations in the water distribution systems. Water uses during pregnancy (ingested tap water, swimming-pool attendance, showers and bath frequencies and duration) were collected with questionnaires. Our results suggest an association between prenatal exposure to DBPs (THMs and HAAs) and the risk of IUGR. No association was found with the risk of preterm birth.
Contexte. La chloration est le traitement le plus utilisé pour désinfecter l’eau distribuée à la population. Lors du traitement, des sous-produits de chloration (SPCs) se forment par réaction de la matière organique naturelle présente dans l’eau avec le chlore. Les produits les plus présents sont les trihalométhanes (THMs), les acides haloacétiques (HAAs). Des études toxicologiques (animales et in vitro) ont montré la génotoxicité et/ou carcinogénicité et la reprotoxicité de certains SPCs. Nous avons mené deux études évaluant les effets sur la santé humaine de l’exposition aux SPCs, dans le domaine du cancer et de la reproduction. Association entre exposition aux THMs et risque de cancer de la vessie : une analyse poolée de 3 études cas-témoins européennes. Cette analyse a inclus 2381 cas et 3086 témoins issus de 3 études cas-témoins (France, Espagne, Finlande). L’exposition environnementale aux SPCs a été mesurée par la concentration en THMs estimée rétrospectivement dans les réseaux de distribution d’eau au cours des 40 années de la fenêtre d’exposition. Les usages de l’eau connus sont l’ingestion, les douches et les bains, la fréquentation de piscine (étude espagnole seulement). Une relation croissante a été observée entre le niveau environnemental de THMs, la durée d’exposition à une eau de surface chlorée et le risque de cancer de la vessie, chez les hommes uniquement. Aucune association n’a été observée avec l’exposition via l’ingestion d’eau du robinet. L’exposition via les douches, les bains et la piscine est apparue liée au risque de cancer de la vessie. Trois études cas-témoins nord-américaines ont été intégrées dans une méta-régression. Aucune spécificité européenne de la relation dose-réponse n’a été mise en évidence. Une relation dose-réponse globale incluant 4351 cas et 7055 témoins a été estimée. Association entre exposition aux SPCs et risque de prématurité et de retard de croissance intra-utérin (RCIU). Cette étude est issue de la cohorte bretonne Pélagie (3400 femmes enceintes recrutées en début de grossesse entre 2002 et 2006). L’exposition pendant la grossesse a été mesurée à l’aide de 2 indicateurs : la concentration en THMs de l’eau distribuée dans les réseaux et le dosage d’un biomarqueur urinaire (acide trichloroacétique) pour un sous-échantillon de femmes (étude cas-témoins nichée). Les niveaux dans les réseaux proviennent de la base de données réglementaire SISE-Eaux. Les usages de l’eau du robinet par les femmes pendant la grossesse ont été collectés par questionnaire (quantité d’eau du robinet bue, fréquentation de la piscine, fréquence et durée des douches et des bains). Notre étude suggère une association entre l’exposition prénatale aux SPCs et le risque de RCIU. Aucune association n’est observée avec le risque de prématurité.

This study aims to describe lead contamination in the homes of children aged 6 months to 6 years in France (without overseas). Between October 2008 and August 2009 ,484 housing units were investigated. Lead in tap water and total and leachable lead levels from floor dust, outdoor soils and paint chips were measured. X-ray fluorescence measurements were carried out on non-metallic and metallic substrates. Nationwide results are provided. The indoor floor dust lead (PbD) geometric mean (GM) was 8.8 microg/m2 (0.8 microg/ft2) and 6.8 mmicrog/m2 (0.6 mg/ft2) for total and leachable lead respectively; 0.21% of homes had an indoor PbD loading above 430.5 microg/m2 (40 microg/ft2). The outdoor play area concentration GM was 33.5microg/kg and 21.7microgkg in total and leachable lead respectively; 1.4% of concentrations were higher than or equal to 400microg/kg. Outdoor floor PbD GM was 44.4 mg/m2 (4.1 mg/ft2) that was approximately 3.2 times higher than the GM of indoor PbD. Lead-based paint (LBP) was present in 25% of dwellings, LBP on only non-metallic substrates was present in 19% of homes and on metallic substrates in 10% of dwellings. The GM of lead concentrations in tap water was below 1 microg/L; 58% of concentrations were lower than 1 microg/L and 2.9% were higher than or equal to 10 mg/L. The age cut-off for homes with lead would be 1974 for paint and 1993 for indoor floor dust. This study provides, for the first time, a look at the state of lead contamination to which children are exposed in French housing. Moreover, it provides policy makers an estimate of the number of French dwellings sheltering children where abatement should be conducted.

Herbicides are generally the most extensively used of the pesticides applied to agricultural crops. However, the literature contains little evidence useful in assessing the potential sources of the general population's exposure to herbicides, including by residential proximity to crops. The objective of this study was to take advantage of data from the PELAGIE mother-child cohort to identify the main determinants of the body burden of exposure to the chloroacetanilide and triazine herbicides commonly used on corn crops in Brittany, France, before 2006. Urine samples from a randomly selected subcohort of women in the first trimester of pregnancy (n=579) were assayed for herbicide metabolites. The residential exposure resulting from proximity to corn crops was assessed with satellite-image-based scores combined with meteorological data. Data on diet, drinking tap water (from the public water supply), occupations, and household herbicide use were collected by questionnaires. Herbicides were quantified in 5.3% to 39.7% of urine samples. Alachlor and acetochlor were found most frequently in the urine of women living in rural areas. The presence of dealkylated triazine metabolites in urine samples was positively associated with residential proximity to corn crops (OR=1.38, 95% CI: 1.05-1.80). Urinary metabolites of both atrazine and dealkylated triazine were correlated with tap water consumption (OR=2.94, 1.09-7.90, and OR=1.82, 1.10-3.03, respectively); hydroxylated triazine metabolites were correlated with fish intake (OR=1.48, 1.09-1.99). This study reinforces previous results that suggest that environmental contamination resulting from agricultural activities may contribute to the general population's exposure to herbicides.

An inter-laboratory exercise was organized under the PHARMAS EU project, by the Advanced School of Public Health (EHESP), in order to evaluate the performances of analytical methods for the measurement of antibiotics in waters (surface and tap). This is the first time such an exercise on antibiotics has been organized in Europe, using different kinds of analytical methods and devices. In this exercise thirteen laboratories from five countries (Canada, France, Italy, the Netherlands and Portugal) participated, and a total number of 78 samples were distributed. During the exercise, 2 testing samples (3 bottles of each) prepared from tap water and river water, respectively, spiked with antibiotics, were sent to participants and analyzed over a period of one month. A final number of 77 (98.7%) testing samples were considered. Depending on substances studied by each participant, 305 values in duplicate were collected, with the results for each sample being expressed as the target concentration. A statistical study was initiated using 611 results. The mean value, standard deviation, coefficient of variation, standard uncertainty of the mean, median, the minimum and maximum values of each series as well as the 95% confidence interval were obtained from each participant laboratory. In this exercise, 36 results (6% of accounted values) were outliers according to the distribution over the median (box plot). The outlier results were excluded. In order to establish the stability of testing samples in the course of the exercise, differences between variances obtained for every type of sample at different intervals were evaluated. The results showed no representative variations and it can be considered that all samples were stable during the exercise. The goals of this inter-laboratory study were to assess results variability when analysis is conducted by different laboratories, to evaluate the influence of different matrix samples, and to determine the rate at which participating laboratories successfully completed the tests initiated.

For groundwater issues (potential leakages in waste repository, aquifer management ...), the development of modeling techniques is far ahead of the actual knowledge of aquifers. This raises two fundamental issues: 1) which and how much data are necessary to make predictions accurate enough for aquifer management issues; 2) which models remain relevant to describe the heterogeneity and complexity of geological systems. The French observatory H+ was created in 2002 with the twofold motivation of acquiring a large database for validating models of heterogeneous aquifers, and of surveying groundwater quality evolution in the context of environmental changes. H+ is a network of 4 sites (Ploemeur, Brittany, France; HES Poitiers, France; Cadarache, France; Campos, Mallorca, Spain) with different geological, climatic, and economic contexts. All of them are characterized by a highly heterogeneous structure (fractured crystalline basement for Ploemeur, karstified and fractured limestone for Poitiers, Cadarache and Mallorca), which is far to be taken into account by basic models. Ploemeur is exploited as a tap-water plant for a medium-size coastal city (15,000 inhabitants) for 20 years. Each site is developed for long term investigation and monitoring. They involves a dense network of boreholes, detailed geological and geophysical surveys, periodic campaigns and/or permanent measurements of groundwater flow, water chemistry, geophysical signals (including ground motions), climatic parameter, etc. Several large-scale flow experiments are scheduled per year to investigate the aquifer structure with combined geophysical, hydrogeological, and geochemical instruments. All this information is recorded in a database that has been developed to improve the sustainability and quality of data, and to be used as a collaborative tool for both site researchers and modelers. This project lasts now for 5 years. It is a short time to collect the amount of information necessary to apprehend the complexity of aquifers; but it is already enough to obtain a few important scientific results about the very nature of the flow heterogeneity, the origin and residence time of water elements, the kinetic of geochemical processes, etc. We have also developed new methods to investigate aquifers (in-situ flow measurements, flow experiment designs, groundwater dating, versatile in-situ probes, etc.). This experience aiming at building up long term knowledge appears extremely useful to address critical issues related to groundwater aquifers: the structure and occurrence of productive aquifer in crystalline basement, the assessment of aquifer protection area in the context of highly heterogeneous flow, the biochemical reactivity processes, the long term evolution of both water quantity and quality in the context of significant environmental changes, for instance.

Résumé : SODACAL prawn farm which was set up as a pilot farm for the development of a prawn industry in New Caledonia, saw its production exponentially rise from 1984 to 1992 (to 400 t/year, then experienced a sharp drop in 1993 (250 t/year) which carried on until 1997, and finally rose back to a stabilizing level around 400 t/year until 1999. The study analyses the difficulties observed before and during the drop of production. Among them, mortality outbreaks, other biological perturbations (benthic mat clearing and crab and fish mortality) and focus on the noxious state of the bottom (reduced and smelling sulfide hydrogen). It particularly shows that Syndrome 93 outbreaks could be observed on this farm from 1990. The analysis highlights the particular climatic conditions in New Caledonia from 1990 to 1995 (drought) and their potential impact on the production. The development of this very particular process of physical and topographic differential evolution of the pond bottom under wind action (accumulation and digging) and its consequences on pond preparation is explained. This work takes a close look at the grow-out management : age and quality of post larvae, pond stocking with pre-grown animals, feed characteristics and management, water exchange management and pumping capacity, strategy on density and size of harvested animals. The results have been assessed using a distinctive tool, the "survival differential", allowing inter-pond comparisons within a site, inter-farms comparisons over pluri-annual periods of time. >From the commented farm results over a 20 year period, we could distinguish the principal factors responsible for the drop of grow-out capacity of some ponds. We have outlined a conceptual description of the different parameters leading to the decrease of survival, an epidemiological scenario and a synoptic table of the history of events and their relative severity. The study draws the hypothesis of a cumulative effect of positive factors, which could have led back to satisfying production results: a physical and biochemical restoration of pond bottoms, a modified and standardized grow-out management, an appropriate pumping capacity and the come back of adequate climatic conditions. This case study illustrates the concept of « flexibility » of the farming capacity and the sustainability of prawn farming in New Caledonia. The clear objective of this 20 year historic investigation was to get relevant information, in order to improve the understanding on the "summer syndrome" which hits two farms at the moment, and to participate in the hypothesis under study by the DAC (2005). We also tried to make the new farmers aware of the fragility of the pond ecosystem, and give some common-sense advices in the hope that such problems will not happen again in other places.
La ferme de SODACAL, conçue à l'origine comme une ferme de démonstration pour le développement d'une filière d'aquaculture exportatrice de crevettes en Nouvelle-Calédonie a connu une phase de croissance exponentielle de sa production de 1984 à 1992 (jusqu'à 400 t/an), puis une chute brutale en 1993 (250 t/an) qui s'est prolongée jusqu'en 1997 et enfin une reprise à partir de 1998 pour arriver à une stabilisation autour de 420 t/an depuis 1999. L'étude inventorie les dysfonctionnements observés avant et pendant la crise de production. Parmi ceux-ci sont évoqués les épisodes de mortalité de crevettes, d'autres désordres biologiques (décollements de tapis benthique et « hécatombes » de crabes et de poissons), l'état délétère des fonds (sédiments réduits et odeur d'hydrogène sulfuré). Elle permet notamment de montrer que des épisodes de mortalité typiques du Syndrome 93 pouvaient être observés sur cette ferme dès 1990. L'étude met en évidence les conditions climatiques remarquables qui ont sévi en Nouvelle-Calédonie de 1990 à 1995 (sécheresse) et leur impact probable sur les élevages. La genèse du phénomène très particulier d'évolution physique et topographique des fonds des bassins par l'effet du vent (accumulation et creusement) et des conséquences sur la préparation des bassins est explicitée. Ce travail aborde la gestion zootechnique : l'âge et la « qualité » des post larves, la pratique des ensemencements à partir d'animaux pré grossis, la gestion de l'alimentation et la nature de la provende, le contrôle du renouvellement d'eau et la capacité de pompage, le choix des densités et du calibre des animaux récoltés. Les résultats ont pu être évalués à travers un outil original «le différentiel de survie », permettant des comparaisons inter-bassins au sein du site, inter-fermes à des périodes pluri annuelles distinctes. A partir des résultats commentés de la ferme obtenus sur 20 années, on a pu distinguer les principaux facteurs qui ont provoqué cette chute d'aptitude culturale de certains bassins. Un schéma conceptuel des différents paramètres ayant conduit à la baisse des survies, un scénario épidémiologique ainsi qu'un tableau synoptique de la chronologie des évènements et de leur relative sévérité sont esquissés. Le rapport ébauche l'hypothèse d'un cumul de causes favorables qui ont permis le retour à des résultats zootechniques satisfaisants : une réhabilitation physique et biochimique des fonds de bassins, une zootechnie modifiée et standardisée, une capacité de pompage adéquate et une climatologie de nouveau favorable. Cette étude de cas permet ainsi d'illustrer les notions d « élasticité » de l'aptitude culturale et de durabilité de la crevetticulture en Nouvelle-Calédonie. Le but de cette enquête historique sur 20 ans est l'acquisition d'informations pertinentes permettant d'améliorer la compréhension du « Syndrome d'été » qui affecte actuellement deux fermes de production et ainsi aider à compléter le scénario hypothétique avancé par le Dac (2005). Enfin, au travers de cet historique, on a cherché également à sensibiliser les nouveaux opérateurs de la filière sur la fragilité de l'écosystème-bassin et présenter quelques conseils de bon sens pour éviter que de tels phénomènes ne se reproduisent ailleurs.

The decline in children's Blood Lead Levels (BLL) raises questions about the ability of current lead poisoning screening criteria to identify those children most exposed. The objectives of the study were to evaluate the performance of current screening criteria in identifying children with blood lead levels higher than 50 µg/L in France, and to propose new criteria. Data from a national French survey, conducted among 3831 children aged 6 months to 6 years in 2008-2009 were used. The sensitivity and specificity of the current criteria in predicting blood lead levels higher than or equal to 50 µg/L were evaluated. Two predictive models of BLL above 44 µg/L (for lack of sufficient sample size at 50 µg/L) were built: the first using current criteria, and the second using newly identified risk factors. For each model, performance was studied by calculating the area under the ROC (Receiver Operating Characteristic) curve. The sensitivity of current criteria for detecting BLL higher than or equal to 50 µg/L was 0.51 (0.26; 0.75) and specificity was 0.66 (0.62; 0.70). The new model included the following criteria: foreign child newly arrived in France, mother born abroad, consumption of tap water in the presence of lead pipes, pre-1949 housing, period of construction of housing unknown, presence of peeling paint, parental smoking at home, occupancy rates for housing and child's address in a cadastral municipality or census block comprising more than 6% of housing that is potentially unfit and built pre-1949. The area under the ROC curve was 0.86 for the new model, versus 0.76 for the current one. The lead poisoning screening criteria should be updated. The risk of industrial, occupational and hobby-related exposure could not be assessed in this study, but should be kept as screening criteria

BACKGROUND: The exposure of children to lead has decreased in recent years, thanks notably to the banning of leaded gasoline. However, lead exposure remains a matter of public health concern, because no toxicity threshold has been observed, cognitive effects having been demonstrated even at low levels. It is therefore important to update exposure assessments. A national study was conducted, in 2008-2009, to determine the blood lead level (BLL) distribution in children between the ages of six months and six years in France. We also assessed the contribution of environmental factors. METHODS: This cross-sectional survey included 3831 children recruited at hospitals. Two-stage probability sampling was carried out, with stratification by hospital and French region. Sociodemographic characteristics were recorded, and blood samples and environmental data were collected by questionnaire. Generalized linear model and quantile regression were used to quantify the association between BLL and environmental risk factors. RESULTS: The geometric mean BLL was 14.9μg/l (95% confidence interval (CI)=[14.5-15.4]) and 0.09% of the children (95% CI=[0.03-0.15]) had BLLs exceeding 100μg/l, 1.5% (95% CI=[0.9-2.1] exceeding 50μg/l. Only slight differences were observed between French regions. Environmental factors significantly associated with BLL were the consumption of tap water in homes with lead service connections, peeling paint or recent renovations in old housing, hand-mouth behavior, passive smoking and having a mother born in a country where lead is often used. CONCLUSIONS: In children between the ages of one and six years in France, lead exposure has decreased over the last 15 years as in the US and other European countries. Nevertheless still 76,000 children have BLL over 50μg/l and prevention policies must be pursued, especially keeping in mind there is no known toxicity threshold.

Although prenatal exposure to water disinfection by-products does not appear to affect the duration of gestation, its impact on fetal growth remains an open question. The authors studied the associations between prenatal exposure to disinfection by-products and fetal growth restriction (FGR) and preterm birth in the PELAGIE cohort, a French birth cohort comprising 3,421 pregnant women recruited between 2002 and 2006. Exposure was assessed by estimating levels of trihalomethanes (THMs) in tap water during pregnancy and maternal water use and by measuring maternal urinary levels of trichloroacetic acid (TCAA) during early pregnancy in a nested case-control design that compared 174 FGR cases, 114 preterm births, and 399 controls. Higher uptake of THMs (especially brominated THMs) was associated with a higher risk of FGR. Women with TCAA detected in their urine (>0.01 mg/L) had a higher risk of FGR than those with TCAA levels below the detection limit (adjusted odds ratio = 1.8, 95% confidence interval: 0.9, 3.7) and had an odds ratio for preterm birth below 1 (adjusted odds ratio = 0.8, 95% confidence interval: 0.3, 2.6). Results from this prospective study, the first to use a biomarker of disinfection by-product exposure, suggest that prenatal exposure affects fetal growth, but the causal agent or agents remain to be identified.
